Your checklist for attending an upcoming appointment at Valley Plastic Surgery

The following measures will ensure the safety, well-being and comfortability of not only yourself, but fellow patients and the team at VPS.

Do NOT attend the clinic if you:

• Are feeling unwell or are experiencing cold or flu-like symptoms. Please contact the clinic ASAP if this applies and we will provide alternate arrangements.

Telehealth appointments are available: (video & phone only)

Please notify staff if you would like to convert your appointment to Telehealth. Please be aware that in some circumstances Telehealth may not be medically advisable.

Clinic Attendance Guidelines:

To protect yourself, other patients, and our team, please follow these measures when attending the clinic:

• Mask Usage: Masks are strongly recommended, especially if you have any mild symptoms following an illness or have been in contact with someone who is unwell.
• Hand Sanitization: Please use the hand sanitization stations provided upon entering and exiting the clinic.
• Physical Distancing: Maintain appropriate physical distancing where possible to ensure everyone’s comfort and safety.

Clinic Behaviour Expectations:

We are committed to maintaining a positive, respectful environment for all patients, staff, and visitors. While attending our clinic, we kindly request that you:

• Treat all staff and fellow patients with respect and courtesy.
• Avoid disruptive behaviour, including loud conversations or inappropriate language.
• Follow the instructions and guidance provided by clinic staff for your safety and the safety of others.
• Always respect the privacy of other patients.

Failure to adhere to guidelines may result in termination of services or being asked to leave the premises. We appreciate your understanding and cooperation in helping us maintain a welcoming and professional environment for everyone.
